Summary:
This Registered Nurse will be responsible for the planning and delivery of healthcare to Veterans within the Home-Based Primary Care (HBPC) program. This Nurse will evaluate Veterans in their homes and develop a Plan of Care in collaboration with the interdisciplinary team. Primary area of coverage is Daytona Beach/Volusia County. However, selectee may be required to see Veterans across six (6) counties covered by the Orlando VAMC. The selectee will be assigned to Daytona Beach PACT. This position requires multiple, daily visits in a Government vehicle to Veterans homes. Specific duties will include:
Functions as a registered nurse in the Veteran's home environment; Evaluates Veterans health, as well as family, and psychosocial history, to identify health care needs and initiates or revises plan of care based on policies of HBPC; Performs accurate physical examinations, using nursing techniques of inspection, palpitation, percussion, and auscultation; Utilizes appropriate instruments and orders and performs relevant diagnostic tests; Provides health teaching to Veterans and their families related to present health problems, prevention of illness, accidents, and health maintenance for them to assume more responsibility for their overall health care; Participates in Veterans and staff education activities, medical and/or nursing research and performance improvement activities; Educates Veterans on prescribed medications and treatments to promote independent medication management; Initiates referrals, consultations, community follow-ups and equipment and supply orders as appropriate; Coordinates the plan of care with other disciplines; Collaborates with clinic team members in providing Veteran care and in the development of clinic program policies and procedures; Coordinates care provided by non-VA home health agencies; and Records patient data and treatment plan, including Veteran's progress notes and responses to care and other duties as assigned. The candidate must be willing to adjust their tour of duty as needed to meet Veteran care needs. The Registered Nurse will report directly to the Nurse Manager for Home Based Primary Care with professional nursing oversight by the Chief Nurse, Geriatrics and Extended Care. Work Schedule:
Normal work days/hours will be Monday-Friday, 7:
30 AM to 4:
00 PM, with work day/hours flexibility required as needs require. Position Requirements:
US Citizenship required; Proficient in spoken and written English language required; Graduate from a National League for Nursing Accrediting Commission (NLNAC) or Commission on Collegiate Nursing Education (CCNE)-accredited program required; Current, full, active and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the US or in the District of Columbia required; A minimum of three (3) years successful, full-time nursing experience; Previous experience caring for ventilator dependent Veterans and Home Care highly desired; Recent experience caring for Veterans in the Hospital or Hospice settings; BLS required. Knowledge/understanding of current VHA standards/care delivery practices for home and community health, as well as those mandated by accrediting/review bodies, such as JCAHO and CAP, and demonstrated skill in successfully incorporating these standards into daily practice; Demonstrated ability to forecast requirements necessary for both short- and long-range planning and model development for more effective, efficient delivery of home care services; Demonstrated ability to be self-directed, function independently, and work effectively with others at all levels within the organization. Experience:
BSN Degree strongly preferred; MSN or master's degree in a related field preferred; National certification in community health or case management preferred; Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
