Social Worker (HUD-VASH Case Manager)

The HUD-VASH Program requires continuous coordination with the community's homeless providers and coalitions as well as with other local, state, and federal agencies.The primary goal of HUD-VASH is to move Veterans and their families out of chronic homelessness. A primary component of the program is VA case management services which are designed to improve Veteran's health and mental health, and to enhance their ability to remain stable, housed, and community-integrated. The Incumbent is responsible for providing case management for a caseload of Veterans in the HUD-VASH program. The Incumbent is responsible for reviewing agreements between Section 8, landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-VASH placements. The Social Worker provides comprehensive discipline-specific services to Veterans and their families. Responsibilities include the management, coordination and provision of discipline-specific services to Veteran and their families in inpatient, outpatient and community settings. In the provision of care in patients from the adult (less than 65 years of age) to the geriatric (greater than 65 years of age), the employee recognizes the age related needs and demonstrates knowledge and care appropriate to that age group. The incumbent is responsible for data collection, administrative record keeping, and monitoring patient or program clinical indicators, thresholds, and performance measurements. The incumbent functions as a Housing Specialist while developing working relationships and directly coordinating with the designated PHA regarding the housing voucher application and award process for each Veterans. The incumbent is responsible to review the agreements between Section 8 landlords, PHAs, and the VA to establish appropriate referral sites for placements. When appropriate, the incumbent assists Veterans with the housing search, completing rental applications, obtaining necessary housing documents, viewing of potential apartments, inspections, lease-ups, and negotiation with landlords. The incumbent assists Veterans with obtaining resources for security deposits, utility expenses, and furnishings. The incumbent also accompanies Veteran participants during PHA appointments and briefings. The incumbent implements, and maintains referral and screening procedures for potential HUD/VASH Veteran participants that meet national policy as well as the needs of medical center mental health services and residential programs. The incumbent is skilled in making referrals, screening and admission criteria recognizes program eligibility, prioritization of subsets of the homeless Veteran population, requirements of the PHAs, and other policy, regulation, or law pertaining to program administration. The incumbent is responsible to ensure criteria and procedures are maintained. The incumbent provides primary input for program participant selection based on structured interviews and clinical assessments. Work Schedule:

30 pm), with rotating tours of duty as needed. The selectee may be required to work or travel between any of the locations within Corporate Orlando, including the new Lake Nona campus. Citizenship. Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part.) Education Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. English Language Proficiency. Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j, this part. In addition to the basic requirements, the GS-11 level requires completion of a minimum of (1) year of post-MSW degree experience in the field of health care social work (VA or no-VA experience) and licensure or certification in a state at the independent practice level. References:

The position requires performance of work which involves light physical effort, such as that involved in lifting light files, bending, standing, reaching, operating office equipment such as telephone, copier, fax machine, scanner, etc. Assignments. This is the full performance level for social workers. GS-11 social workers are licensed or certified to independently practice social work. They are assigned to all program areas, including inpatient or outpatient medicine, surgery, mental health, neurology, rehabilitation medicine, and geriatrics. Employees provide professional, independent social work services in the assigned area. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience. The full performance level of this vacancy is GS-11.
